Service Agreement
Special Pathogens Laboratory (SPL)’s maximum liability relating to services rendered under this Service Agreement, regardless of the form of action, shall be limited to the charges paid to Special Pathogens Laboratory for the services giving rise to the liability. Payment shall be NET 30 days from date of invoice unless otherwise agreed in writing. Any balance over the 30 days will be subject to a 1.5% per month finance charge (18% per annum). Client shall also assume responsibility for all collection costs, including finance charges, legal expenses and attorney fees. SPL shall subcontract the test(s) to an approved lab if SPL is unable to perform the test(s). Your acceptance of these terms shall constitute your approval for us to do so. If you are a user with administrator permissions, you understand that you are authorized to make any updates to the account at any time, and SPL shall not be responsible for the release of reports or any data to unintended contacts. I (We) assume responsibility for and guarantee payment to SPL of all sums due and payable by the party listed on this form, including interest, late charges, and collection costs.
